Form Recognition tool not recognizing/creating some text input field types but recognizing others that look the same

I am testing to see if this software will fit my needs and when I take one of my PDF documents and run Form Recognition on it only some of the text input fields are detected but not others that are basic fields. For example, it creates a fillable Zip Code field but does not create fields for Last Name or First Name. Check out the attachment if you want to see what I'm talking about

No there is no data on these forms yet, I want to make them editable so that I can actually import data into them. So that's why I need these form fields created. I just don't get why only some fields are created and others aren't even on the same line with the same appearance and using what I would consider standard identifying labels like Address or Last Name, etc.
